Each parent works with one Care coach who offers them wrap- around support and treatment. That would not be possible without a strong, integrated, collaborative Care team wrapping around that coach with support.

Each Care team is made up of Care coaches, a supervisor and a community access navigator. Teammates offer each other ideas, supervisors offer oversight and professional development, and the community access navigator brings in community support and donations.
